Like maybe...McCoy has pulled Spock into this kiss all of a sudden and Spock's taken by surprise, and the Vulcan has his hand up in that two-fingered gesture because he was going to give McCoy a Vulcan-kiss but McCoy being McCoy was all NOPE and grabbed him by the neck and pulled him forwards. Or just those two sitting beside each other, with their backs to the viewer and their hands clasped in a nervous/embarrassed way. Or Spock laying on a medbay bed and the medbay's quite dark or something and McCoy's there looking sleepy and worried, holding the Vulcan's hand.
